It\u2019s more of a strange feeling of instability, like walking on a mattress or on clouds\u2014a dizziness that sets in when you move your neck and is almost always accompanied by stiffness and neck pain. Anyone who experiences it knows it\u2019s real and bothersome. And yet, it\u2019s one of the most debated topics in the doctor\u2019s office.   <\/p>\n\n<p class=\"wp-block-paragraph\">Let&#8217;s explain this thoroughly and without causing alarm\u2014but also with an important caveat up front, because caution is not optional here\u2014.<\/p>\n\n<h2 class=\"wp-block-heading\">What is cervical vertigo (and why does it spark so much debate)?<\/h2>\n\n<p class=\"wp-block-paragraph\">Cervical dizziness, or cervicogenic dizziness, is a sensation of <strong>imbalance and instability<\/strong> associated with neck dysfunction or pain. Note a key distinction: it\u2019s not usually the typical rotational vertigo (that sensation of the room spinning), but rather lightheadedness, a feeling of unsteadiness when moving, or the sensation of \u201cwalking on soft ground.\u201d <\/p>\n\n<p class=\"wp-block-paragraph\">Its defining characteristic\u2014and the reason for the debate\u2014is that <strong>it is a diagnosis of exclusion<\/strong>. There is no test that definitively confirms it. It is reached <em>only after<\/em> ruling out, one by one, other causes of dizziness. In fact, its very existence\u2014and the extent to which the neck is responsible\u2014remains a subject of scientific debate. There are <strong>much more common and better-defined<\/strong> causes of dizziness and vertigo that should be ruled out first: <\/p>\n\n<figure class=\"wp-block-table\"><table>\n<thead><tr><th>Reason<\/th><th>As is usually the case<\/th><th>Distinctive clue<\/th><\/tr><\/thead>\n<tbody>\n<tr><td><strong>VPPB (crystals)<\/strong><\/td><td>Intense, very brief (seconds) rotational vertigo<\/td><td>It goes off when you turn over in bed or look up<\/td><\/tr>\n<tr><td><strong>Meniere&#8217;s Disease<\/strong><\/td><td>Episodes ranging from 20 minutes to several hours<\/td><td>With hearing loss, tinnitus, and a feeling of fullness in the ear<\/td><\/tr>\n<tr><td><strong>Vestibular neuritis<\/strong><\/td><td>Intense and constant vertigo, days<\/td><td>After a viral infection, with nausea<\/td><\/tr>\n<\/tbody><\/table><\/figure>\n\n<p class=\"wp-block-paragraph\">BPPV (the so-called \u201clittle crystals\u201d in the inner ear) is the most common cause of vertigo, and it has its own treatment. The difference from cervical vertigo is that, in the latter, the instability is <strong>associated with neck pain<\/strong> and worsens with neck movements, rather than with changes in the position of the head in space. <\/p>\n\n<p class=\"wp-block-paragraph\">Practical conclusion: If you&#8217;re experiencing dizziness, the first step isn&#8217;t to &#8220;treat your neck,&#8221; but to undergo a thorough evaluation to rule out ear problems and other causes. Your neck, especially the upper part, is full of these sensors. <\/p>\n\n<p class=\"wp-block-paragraph\">The theory is this: if the joints and muscles in the neck aren&#8217;t working properly or are painful, they send &#8220;distorted&#8221; information to the brain. That signal conflicts with the (correct) information coming from the eyes and ears. The brain receives conflicting versions of reality and, unable to reconcile them, generates that sensation of dizziness and unsteadiness.  <\/p>\n\n<blockquote class=\"wp-block-quote cqo-dato is-layout-flow wp-block-quote-is-layout-flow\">\n\n<p class=\"wp-block-paragraph\"><strong>Did you know&#8230;?<\/strong> The upper part of the neck is one of the areas with the most position receptors in the entire body. Before attributing anything to the neck, there are signs that warrant a medical evaluation to rule out potentially serious problems (such as a circulation problem in the neck arteries): <\/p>\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n\n<p class=\"wp-block-paragraph\"><strong>\u26a0\ufe0f Seek medical attention if dizziness is accompanied by:<\/strong><\/p>\n\n\n<ul class=\"wp-block-list\">\n\n<li><strong>Double vision<\/strong>, difficulty <strong>speaking<\/strong> or <strong>swallowing<\/strong>.<\/li>\n\n\n<li><strong>Sudden falls<\/strong> without losing consciousness.<\/li>\n\n\n<li>Numbness in the face or limbs, loss of strength, or lack of coordination.<\/li>\n\n\n<li>A sudden, very severe headache\u2014&#8221;the worst of your life.&#8221;<\/li>\n\n\n<li>Fever, unexplained weight loss, or constant nighttime pain.<\/li>\n\n<\/ul>\n\n<\/blockquote>\n\n<p class=\"wp-block-paragraph\">These symptoms (which professionals summarize as &#8220;the 5 Ds&#8221;: <em>dizziness, diplopia, dysarthria, dysphagia, and drop attacks<\/em>) are not something to be treated on an examination table; they must first be evaluated to rule out any serious conditions.<\/p>\n\n<h2 class=\"wp-block-heading\">Treatment: physical therapy, once other causes have been ruled out<\/h2>\n\n<p class=\"wp-block-paragraph\">Once vestibular, neurological, and vascular causes have been ruled out and everything points to the neck, treatment focuses on that area. The evidence is still limited\u2014let\u2019s be honest\u2014but promising, and it combines: <\/p>\n\n<ul class=\"wp-block-list\">\n\n<li><strong>Manual therapy<\/strong> for the cervical and thoracic spine to reduce pain and stiffness.<\/li>\n\n\n<li><strong>Therapeutic exercise<\/strong>: neck motor control, proprioception, balance, and eye-head coordination. It is a central component, not a supplement. <\/li>\n\n\n<li><strong>Education<\/strong>: Understanding what&#8217;s happening, overcoming the fear of movement, and maintaining good posture.<\/li>\n\n<\/ul>\n\n<p class=\"wp-block-paragraph\">What isn&#8217;t honest is promising an immediate cure. Now you know two things. First, that neck issues <em>can<\/em> be behind dizziness, through that \u201cconflict of signals\u201d with your sense of balance. Second, and most importantly: cervical dizziness is the last thing to be confirmed, not the first thing to be assumed.
