It doesn&#8217;t hurt. It doesn&#8217;t show itself. Osteoporosis works silently for years, hollowing out the bones from the inside without you noticing a single thing \u2013 until one day, with a silly fall or a wrong move, a bone breaks. That&#8217;s why it&#8217;s called &#8220;the silent disease&#8221;. And that&#8217;s why understanding it in time, before the first fracture, changes everything.<\/p>\n\n<p class=\"wp-block-paragraph\">An honest clarification from the outset: osteoporosis is a medical matter. Its diagnosis and the drugs are managed by your doctor. Osteoporosis appears when that balance tips and more bone is lost than is formed. The result is <strong>more porous and fragile bones<\/strong>, with a greater risk of breaking.<\/p>\n\n<p class=\"wp-block-paragraph\">It&#8217;s &#8220;silent&#8221; because that loss produces no symptoms. Many people discover they have it on the day they suffer a fracture from a minor knock that, with healthy bones, wouldn&#8217;t have amounted to more than a scare.<\/p>\n\n<h2 class=\"wp-block-heading\">Who is most at risk?<\/h2>\n\n<p class=\"wp-block-paragraph\">Some factors can&#8217;t be changed; others can:<\/p>\n\n<figure class=\"wp-block-table\"><table>\n<thead><tr><th>Non-modifiable<\/th><th>Modifiable \/ medical<\/th><\/tr><\/thead>\n<tbody>\n<tr><td>Being a woman (especially after the menopause)<\/td><td>Calcium and vitamin D deficiency<\/td><\/tr>\n<tr><td>Advanced age<\/td><td>A sedentary lifestyle<\/td><\/tr>\n<tr><td>Family history (hip fracture)<\/td><td>Tobacco and excess alcohol<\/td><\/tr>\n<tr><td>A slim build<\/td><td>Prolonged use of corticosteroids<\/td><\/tr>\n<\/tbody><\/table><\/figure>\n\n<p class=\"wp-block-paragraph\">Added to this are certain conditions (coeliac disease, kidney or liver problems, rheumatoid arthritis \u2026) that also increase the risk.<\/p>\n\n<blockquote class=\"wp-block-quote cqo-dato is-layout-flow wp-block-quote-is-layout-flow\">\n\n<p class=\"wp-block-paragraph\"><strong>Did you know \u2026?<\/strong> The drop in oestrogen after the menopause speeds up bone loss. So-called fragility fractures happen with minimal trauma \u2013 a fall from your own height \u2013 and concentrate in the <strong>hip, wrist and vertebrae<\/strong>.<\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Vertebral<\/strong> ones are especially treacherous: sometimes they occur without a clear blow and show up as sudden back pain, loss of height or that <strong>stooped posture<\/strong> (&#8220;hump&#8221;). <strong>Hip<\/strong> ones are the most serious, usually require surgery and can cost you your independence. That&#8217;s why the goal of all treatment is, at heart, <strong>to stop you breaking<\/strong>.<\/p>\n\n<h2 class=\"wp-block-heading\">How is it diagnosed?<\/h2>\n\n<p class=\"wp-block-paragraph\">The reference test is <strong>bone densitometry (DEXA)<\/strong>: it measures the bone mineral density at the hip and spine. It&#8217;s painless and with very little radiation. The result is given as a <strong>T-score<\/strong>; a value of \u22122.5 or lower indicates osteoporosis. In addition, tools like <strong>FRAX<\/strong> estimate your 10-year fracture risk by combining several factors. The person who decides and interprets all this is the doctor.<\/p>\n\n<h2 class=\"wp-block-heading\">Treatment: medicine, calcium, vitamin D \u2026 and exercise<\/h2>\n\n<p class=\"wp-block-paragraph\">There&#8217;s no miracle &#8220;cure&#8221;, but osteoporosis is managed very well to prevent fractures. What works: <strong>strength and balance exercise<\/strong>; <strong>reviewing medication<\/strong> that may cause dizziness or drowsiness; <strong>looking after your eyesight<\/strong>; and a <strong>safe environment<\/strong> at home (out with loose rugs, good lighting, grab rails in the bathroom, non-slip footwear).<\/p>\n\n<h2 class=\"wp-block-heading\">The role \u2013 and the limits \u2013 of physiotherapy<\/h2>\n\n<p class=\"wp-block-paragraph\">This is where a good exercise plan adds a great deal: physiotherapy helps with <strong>strength, balance, posture and pain<\/strong> (for example, after a vertebral fracture). But we have to be honest and prudent:<\/p>\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n\n<p class=\"wp-block-paragraph\"><strong>\u26a0\ufe0f Precautions in osteoporosis, especially if it&#8217;s severe:<\/strong><\/p>\n\n\n<ul class=\"wp-block-list\">\n\n<li>Avoid <strong>high-impact<\/strong> exercises (jumping) and abrupt <strong>flexion or twisting<\/strong> movements of the spine.<\/li>\n\n\n<li>Avoid <strong>high-velocity spinal manipulations<\/strong>, because of the fracture risk.<\/li>\n\n\n<li>Every programme must be <strong>supervised by a qualified professional<\/strong>.<\/li>\n\n<\/ul>\n\n<\/blockquote>\n\n<p class=\"wp-block-paragraph\">And, above all: manual therapy or exercise <strong>don&#8217;t &#8220;cure&#8221; osteoporosis<\/strong>. They are an \u2013 important \u2013 part of a plan led by the medical team.<\/p>\n\n<h2 class=\"wp-block-heading\">Warning signs and when to see a doctor<\/h2>\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n\n<p class=\"wp-block-paragraph\"><strong>\u26a0\ufe0f See a doctor if:<\/strong><\/p>\n\n\n<ul class=\"wp-block-list\">\n\n<li>You&#8217;ve broken a bone from a <strong>minor<\/strong> knock or fall (fragility fracture).<\/li>\n\n\n<li>You have a <strong>sudden, intense back pain<\/strong>, especially if you&#8217;re older (possible vertebral fracture).<\/li>\n\n\n<li>You&#8217;ve <strong>lost height<\/strong> (more than 2\u20133 cm) or a &#8220;hump&#8221; is forming.<\/li>\n\n<\/ul>\n\n<\/blockquote>\n\n<p class=\"wp-block-paragraph\">It&#8217;s also worth speaking to a doctor if you had an early menopause, have taken corticosteroids for months, one of your parents fractured a hip, or you&#8217;ve simply turned 50 and want to assess your risk.<\/p>\n\n<h2 class=\"wp-block-heading\">In short<\/h2>\n\n<p class=\"wp-block-paragraph\">Let&#8217;s go back to that idea from the start: osteoporosis gives no warning. But that doesn&#8217;t mean you can&#8217;t do anything \u2013 quite the opposite. Between timely densitometry, calcium and vitamin D, a good plan of strength and balance exercise, a safe home and, when appropriate, the treatment the doctor prescribes, there&#8217;s an enormous margin to reach old age with strong bones and no fractures.<\/p>\n\n<p class=\"wp-block-paragraph\">Bone responds to stimulus at any age.
